In the beginning ER was a show mainly about medical heroes fighting the good fight against disease, injury, red tape, budgets and so on. The heroes had flaws, sure. They had some problems, they made some mistakes, every now and then they made TRAGIC mistakes. But overall they were all at least reasonably competent (some were super-competent) and trying hard.
Several years ago, however, they made a major switch of emphasis. Instead of the main focus being on medicine, the main focus became "screw-ups as doctors". Almost all new incoming doctors over several years had SERIOUS problems. Dealing with those problems pushed "the good fight" into the background.
At the same time, it simply became hard to understand what everyone was saying in the ER rooms. I've gone back and watched some early episodes - man, what a difference. You can actually HEAR what each person is saying. They spoke relatively slowly and clearly and rarely talked on top of each other. In later seasons you might have two, three or even four people talking quickly at once in a real verbal mishmash.
So yeah, I think they had a major deal with some fish-farm to keep supplying sharks in wholesale quantities.
